Night bot sometimes randomly (has happened multiple times) add a command “to” without anyone actually adding the command. is there a way to fix this?

Nightbot doesn’t add commands if you don’t ask it to.
What is the message telling you that a command is added?
I have my suspicions on what it could actually be, but I want to be sure.
